Dear Parent,

Thank you for completing your child’s Intent to Enroll Application and parent meeting. To secure your child’s placement for the 2014-2015 school year, you must complete the official registration paperwork required by Shelby County Schools.

Registration paperwork can be picked up at Miracle Temple Ministries (3750 Millbranch Rd) Monday-Friday between 9am-3pm or you may print it online, www.visionprep.org/registration.
The following documentation must accompany the registration packet:
 
  • A copy of student’s social security card
  • A certified copy of student’s birth certificate (Kindergarten students must be 5 years old on or before August 15, 2014)
  • TN Dept of Health Immunization Certificate
  • 2 documents showing Proof of Residence (parents who live with another person must provide a notarized , which is provided in the registration packet):
    • TN driver’s license
    • Current MLGW bill
    • Copy of lease or mortgage statement
    • Copy of public assistance/government benefits
    • Copy of real estate property tax receipt
  • Rising 1st grade students must have a copy of final report card showing promotion to first grade.
  • Kindergarten students must have proof of a physical exam within 12 months prior to enrollment.
  • Students currently receiving special education services must provide a recent copy of an IEP or 504 plan.
If your child’s paperwork is not completed by Wednesday, July 23rd, your child will be placed on our waiting list. Our waiting list procedure is included for your review.

If you’ve made the decision to place your child at another school, feel free to contact us so we may remove your child from our enrollment list.

Waiting List Procedure
 

  1. A student is placed on the waiting list if the parent completes the application process but fails to complete the required enrollment paperwork by the requested deadline (July 23, 2014)or completes an Intent to Enroll application after all available placements are filled.
  2. If a student stops attending Vision Prep or declines admission, the next available student on the waiting list for that grade will be offered admission until the vacant seat is filled.
  3. No student will be admitted ahead of other eligible students on the waiting list unless said student is either a sibling of an enrolled student.
  4. Students on the waiting list will be contacted by phone with any offer of admission to the school. Parents will have 1 business day to accept the admission offer. It is the parent/guardian’s responsibility to notify the school with any change of contact information.
  5. Any student who is offered admission to Vision Prep and declines admission will need to reapply.
  6. Students who completed the application process but failed to complete the required enrollment paperwork by July 23, 2014 will be added to the bottom of the existing Waiting List in the order his/her Intent to Enroll application was received. The student’s name will remain on the Waiting List until his/her name comes to the top of that grade’s waiting list and an opening in his/her grade is available.
  7. Vision Prep will maintain a rolling waiting list from year to year. The waiting list will advance all students one grade level each year; students only need to be placed on the waiting list once.
  8. Students who are on the waiting list as non-siblings and become a sibling, due to the acceptance and attendance of their brother/sister, will be given sibling status. It is the parent’s responsibility to notify the school if this occurs. If a student currently on the waiting list changes his/her grade, it is the parent’s responsibility to notify the school. At that time, the student will be placed in the next available slot at the end of the waiting list of the new grade.
  9. Vision Prep will keep accurate and secure electronic and paper records of the waiting list. The waiting list will contain the names, home addresses, telephone numbers, grade levels, date of birth, and sibling status of students placed on the waiting list but did not gain admission.
