A Note From The Project Coordinator

Register today for the first ever POLST training on May 6, 2015. This class is a phenomenal opportunity for individuals to help the mission of Light the Legacy and earn CEU's!

The training will take place at Healthpartners Central Minnesota Clinics in Sartell.
Have a speaker come to your group!

We invite all groups including businesses, religious organizations, university groups, and any other group that would like a trained and certified advance care planning facilitator visit with them. It’s also helpful for your group to complete a health care directive prior to the session, and Light the Legacy can help you with that as well. New Year, New Horizons

Each new year there are new challenges, new adventures, new excitement, and new blessings. As we head into the second month of this New Year, we continue to be amazed at the work we’ve done so far, and the plans we have for the future. The past successes and the ability to forge ahead is due to the people who support us with their time, their talents, and their financial support so that we are able to continue the mission of sharing end of life care to all residents of Central Minnesota. Some of the things we are working on are incremental improvements. Others are major changes that will help launch Light the Legacy into a new non-profit status and a well-known organization that fulfills its purpose. We thank you all again for all you do and appreciate your support. I hope you are having a wonderful start to 2015.

New POLST Class
For the very first time, Light the Legacy is proud to sponsor a POLST training class. POLST or Physician Order for Life-Sustaining Treatment is a nationally recognized form that supports physicians speaking with patients to complete advance care directives and encourages them to create medical orders that can be carried out by health care workers in emergency situations. Developed by Gundersen Health and facilitated by Respecting Choices® certified instructors Lynn MacKenzie & Patricia Bresser, the training will take place on Tuesday, May 6th. Registration should be completed as soon as possible as space for the class will fill quickly.  The training will be at HealthPartners Central Minnesota Clinic and is a phenomenal way for individuals to be trained and certified in the POLST paradigm. Nurses will receive 17.8 CEU’s and social workers will receive 16.4 at the completion of the program. We hope to see you there!

REGISTER NOW - ACP Facilitator Training, March 17th

This training is phenomenal and a comprehensive look at how you can assist others with advance care planning and health care directives. Becoming a certified facilitator allows you to change the lives of so many you come in contact with. CEU’s are also available for completion of this class. 

Speakers Available
You may have noticed the addition to our website the ability to request a speaker come to your group to discuss end of life care, health care directives, and assist individuals completing them. Light the Legacy would be honored to speak at your business, place of worship, or University organization about Advance Care Planning. This is an excellent way to become more familiar with this very important subject, and also have the ultimate goal of completing your own Health Care Directive. National Health Care Decisions Day
It’s amazing to consider, but April is coming fast. National Healthcare Decision Day is April 16, 2015. This day was dedicated to help the millions of Americans that do not have a health care directive in place. The day is a collaborative effort that involves both state and local organizations across the country to increase awareness, rally together, and mobilize those working towards greater use of Health Care Directives in their areas. Free Grief Program
Helps When It Hurts is an event that is FREE and OPEN TO THE PUBLIC. The event will be held on Saturday, Feb. 21st from 10:00 a.m. – Noon at the CentraCare Health Plaza’s Windfeldt Room. The event will feature a grief webinar presented by the Hospice Foundation of America and special guest speaker Roxann Storms, MSW, LICSW. You can also view the flyer here for more information.
Being Mortal Trailer
On Feb. 10, 2015, FRONTLINE follows renowned New York writer and Boston surgeon Atul Gawande as he explores the relationships doctors have with patients who are nearing the end of life. In conjunction with Gawande's new book, "Being Mortal," the film explores how the medical profession can better help people navigate the final chapters of their lives with confidence, direction and purpose. HCD Short Form Translations
Honoring Choices and the Twin Cities Medical Society have provided translations of the health care directive short form in several languages. With a growing population of non-English speaking residents it has become more and more important to make resources available to them. The translated forms can be found here
