A Note From The Director

Have a speaker come to your group!

We invite all groups including businesses, religious organizations, university groups, and any other group that would like a trained and certified advance care planning facilitator visit with them. It’s also helpful for your group to complete a health care directive prior to the session, and Light the Legacy can help you with that as well. For more information or to request a speaker, please contact Lynn MacKenzie at lynn@lightthelegacy.org.
FIRST EVER!

Register today for the exceptional POLST training on October 8, 2015. This class is a phenomenal opportunity for individuals to help the mission of Light the Legacy and earn CEU's! This class is expected to fill up fast so register now to confirm your seat.

It's Summer!!

Hello, friends and supporters of Light the Legacy. The time continues to roll on. As the days get longer, and the temperature gets warmer, we continue to push forward with our mission. Currently, the board is working on making a better impact in all we do, and we are still ironing out all the necessary details. We are hoping to be officially a non-profit very soon, and we can't wait. The change this organization has witnessed and the accomplishments we have reached are amazing. This is not to boast. Instead, it is to thank you so much, each one of you for your support, your time, and the many other contributions that have made this possible. With each passing day, we move closer to our goal, and it is often challenging to sit back and reflect in all the hustle and bustle of life. Summer is an excellent time just to take a moment, on a trail in the woods, in a hammock by the beach, or on a lawn chair in your own backyard. Reflect on the things you have accomplished, consider where you want to go and chart a course to get there. That is what we are doing at Light the Legacy. When I take that moment to reflect on our past, I am simply amazed and humbled by all of you who made it possible. As I look to the future, I am confident knowing you are by our side. Let's all make this summer unforgettable. Capture every moment, enjoy it, and don't get so wrapped up in life that you miss it. Thank you all again, and I honestly hope that your summer will be exceptional. 
 

Save The Date - July 16, 2015

Honoring Choices Minnesota is sponsoring the annual "Sharing the Experience" conference at the Ramada Plaza Hotel in downtown Minneapolis. This event is a phenomenal opportunity that you do not want to miss. The conference provides an opportunity to network with others in the field, develop new skills, find new resources, and be inspired by leaders in the cause. If you are an Advance Care Facilitator, a medical professional, or someone who is passionate about end-of-life care, this conference will be well worth it to attend. Keynote speakers Kris Maser, JD and Ruth Bachman, will inspire you and equip you to make a larger impact in your sphere of influence. Light the Legacy will be a vendor at the conference as well, so be sure to stop by and say hello! More information and registration is available on the conference webpage. University of Minnesota Offers New Curricula

The University of Minnesota has partnered with Honoring Choices Minnesota to create an online curriculum entitled "Honoring Choices Across Cultures: A Short Course". The content is designed to assist in the often difficult conversations during advance care planning for caregivers, patients, and family members. The course targets individuals of all kinds including physicians, facilitators, nurses, chaplains, social workers, and many others. Up to 4.5 AMA PRA Category 1 Credits are available. This is an exciting development, and we appreciate the University of Minnesota as well as Honoring Choices for creating such an excellent opportunity. One designed to increase the knowledge of those who deal with advance care planning decisions on a regular basis. To register, just download the appropriate form for your class and return it with your payment. We will confirm your registration and provide you with the online curriculum to complete before the class. These classes are excellent and will help you and your conversations regarding advance care planning immensely. Classes do fill up fast so don't delay. Submit your registration today! Expo For Seniors On August 15th, 2015

This event will be held at the River's Edge Convention Center in downtown St. Cloud from 8:00 to 11:30. You won't want to miss the opportunity to participate in St. Cloud's largest event geared for the older adult population! This targeted event will bring you face-to-face with over 1,000 people. This year’s theme will be Senior Safety. Come and join 100+ vendors in showcasing products, services and information geared toward seniors. Light the Legacy will be there and will have certified advance care planning facilitators to talk to about advance care planning and be available to answer questions. New this year, they are switching from a free lunch to a free breakfast!  Breakfast will be served from 7:30 to 9:00 am in Terry Haws B room at the Civic Center. Lori Swanson, Minnesota Attorney General, will be the keynote speaker. More information can be found on the event website
 

Light the Legacy In Action - TPT Documentary
 
Honoring Choices: Health and Faith in MN is a documentary created by Twin Cities Public Television and Honoring Choices Minnesota. This documentary features our own local influences. Light the Legacy is represented by Dr. Merryn Jolkovsky, Michele Rosha, Patty Bresser, Lynn MacKenzie and Michelle Trelfa. See firsthand the impact we are making in central Minnesota! You can view the documentary on TPT (channel 2) at the following dates and times:

The documentary is also available anytime on our homepage as well as the Honoring Choices website. This is a powerful documentary that should not be missed.

Light The Legacy is a community-based organization dedicated to improving end-of-life care in Central Minnesota. Our organization hopes to inspire ongoing advance care planning conversations among family members and friends, between patients and health care providers, and in the community at large. Our goal is to encourage the people of central Minnesota to make informed health care decisions and to implement programs that facilitate patient- and family-centered end-of-life care.
