Request for Proposals: Addressing HIV Care and Housing Coordination through Data Integration to Improve Health Outcomes along the HIV Care Continuum 

On behalf of the Health Resources and Services Administration’s HIV/AIDS Bureau and the U.S. Department of Housing and Urban Development’s Office of HIV/AIDS Housing, we present the Request for Proposals (RFP) announcement by the Rand Corporation, the Coordination and Technical Assistance Center for the initiative known as Addressing HIV Care and Housing Coordination through Data Integration to Improve Health Outcomes along the HIV Care Continuum. This RFP solicits applications for a new initiative funded through the Secretary’s Minority AIDS Initiative Fund (SMAIF) to support a minimum of four (4) performance sites to support the electronic integration of housing and HIV care data systems and the development of interventions to improve health and housing outcomes. The deadline for applications is Dec. 15, 2015.

Deadline: December 15, 2015

Purpose: This RFP announcement solicits applications to support a minimum of four (4) performance sites for a new initiative entitled Addressing HIV Care and Housing Coordination through Data Integration to Improve Health Outcomes along the HIV Care Continuum. This initiative seeks to support the electronic integration of housing and HIV care data systems in the coordination of housing and HIV care services to improve health outcomes along the HIV Care Continuum, especially among racial and ethnic minority persons living with HIV who are unstably housed, at risk for, or experiencing homelessness. A minimum of four (4) performance sites will be selected following in-person site visits from the project’s Coordination and Technical Assistance Center (CTAC) with candidate sites. The funding period for performance sites will be over two years, from an estimated start date of May 1, 2016 to August 31, 2018, to allow time for implementation and evaluation.

Proposal Assistance Webinar: All interested applicants are encouraged to participate in a proposal assistance webinar for this performance site funding opportunity. The purpose of this webinar is to assist potential applicants in preparing applications that address the requirements of this funding announcement. Participation in a pre-application TA webinar is optional and will not impact final site selection.
