We are pleased to announce and strongly encourage your review of a recently released, action oriented white paper entitled, HIV Care Continuum: The Connection between Housing and Improved Outcomes along the HIV Care Continuum. This resource was created by the Department of Housing and Urban Development’s (HUD) Office of HIV/AIDS Housing to assist Housing Opportunities for Persons with AIDS (HOPWA) and Ryan White HIV/AIDS Program (RWHAP) grantees in their collaborative use of the HIV Care Continuum.
Acknowledged by Executive Order in July of 2013, the HIV Care Continuum is an innovative framework used by federal, state, and local agencies to identify opportunities to improve the delivery of HIV care and resulting health outcomes. The white paper outlines each step of the HIV Care Continuum (Diagnosis, Linkage to Care, Retention in Care, Prescribed ART, and Viral Suppression), explores the impact of housing status along the continuum, and calls for strategic partnerships between health care and housing providers to address significant gaps in care for unstably housed people living with HIV/AIDS (PLWH).
As noted in the paper, leveraging partnerships aimed at improving health outcomes for unstably housed PLWH at each stage of the HIV Care Continuum is critical.
Accelerating improvements in health outcomes for all PLWH is a national priority. HIV Care Continuum: The Connection between Housing and Improved Outcomes along the HIV Care Continuum is an excellent resource for organizations seeking to initiate or expand efforts to integrate HIV care and housing assistance at various steps along the HIV Care Continuum.
