NAM report highlights PROMIS

A new consensus study report released by the National Academies of Sciences Engineering and Medicine (NAM) brings focus on PROMIS as a sustainable structure for common measurement. The report highlights commissioned paper, Ontologies in Patient Reported Outcomes: Conceptual issues and challenges addressed by the Patient-Reported Outcomes Measurement Information System (PROMIS), by David Cella and Ron D. Hays. The full NAM study report, Ontologies in the Behavioral Sciences: Accelerating Research and the Spread of Knowledge is available for free download.

Towards standardization of measuring anxiety and depression: Differential item functioning for language and Dutch reference values of PROMIS item banks

Elsman EBM, Flens G, de Beurs E, Roorda LD, Terwee CB. PLoS One. 2022 Aug 23.eCollection 2022. https://doi.org/10.1371/journal.pone.0273287

Cross-cultural validity of PROMIS measures is essential for the dissemination of standardized measurements across the globe. The authors of this study evaluated the cross-cultural validity of PROMIS Anxiety and Depression item banks between the United States and the Netherlands. Differential item functioning of all items in both item banks was tested based on data from the Dutch general population and PROMIS wave 1. McFadden’s pseudo R2-change of 2% was used as the critical threshold. In both item banks, two items had DIF but with minimal impact on population level T-scores for full item banks and short forms. The authors concluded that the limited number of items with DIF and its minimal impact, enables the use of standard (US) item parameters and comparisons of scores between Dutch and US populations.

A Patient Reported Outcome Ontology: Conceptual Issues and Challenges Addressed by the Patient-Reported Outcomes Measurement Information System® (PROMIS ®)

Cella D, Hays RD. Patient Relat Outcome Meas. 2022 Aug 15. eCollection 2022. https://doi.org/10.2147/prom.s371882

The authors briefly review the history of measuring perceptions of health and quality of life, followed by an examination of conceptual issues related to terminology that have led to potentially conflicting ontologies. Challenges posed by the lack of consensus on common meaning and the proliferation of measures are discussed. The ontology adopted PROMIS is suggested as a potential solution. In addition, the authors discuss issues associated with mapping the PROMIS domain framework onto other familiar ontologies and recommend a way forward for PROMIS to provide a sustainable ontological structure to enable coherent common measurement.

Evaluation of PROMIS Cognitive Function Scores and Correlates in a Clinical Sample of Older Adults

Edelen MO, Harrison JM, Rodriguez A, Weir R, Lai JS, Langer MM, Hanmer
J. Gerontol Geriatr Med. 2022 Aug 13. eCollection 2022.

Mild cognitive impairment is an early warning sign of dementia. In this study the authors assessed the utility of self-reported cognitive function using two PROMIS Cognitive Function (PROMIS-CF) items in an observational clinical sample of patients aged 65 and older (n = 16,249). The association of PROMIS-CF scores with clinical characteristics and Montreal Cognitive Assessment (MoCA) scores was evaluated, and logistic regression was used to examine predictors of 1-year decline in PROMIS-CF scores among patients with available data. PROMIS-CF scores were associated with lower (more impaired) scores for patients with cognitive impairment (CI) diagnoses, multiple comorbidities, and those taking cognitive enhancing or interfering medications. PROMIS-CF scores were also positively associated with MoCA scores. Predictors of 1-year decline in PROMIS-CF scores included CI diagnoses, use of cognitive enhancing medications, higher depression scores, and lower social role function. The authors conclude that PROMIS-CF items may be used in a brief patient-administered screening tool for CI.
