Greetings! It’s been a busy spring so far. We enjoyed a wonderful conference at the 2013 ILHIMA Annual Meeting in April and have been working on a variety of consulting engagements from Montana, to Colorado, California and our home state of Illinois. 
 
Providers, who are continually dealing with the impact of regulatory changes coupled with the need to plan, develop a strategy and implement an EHR or Computer Assisted Coding system, often face tremendous challenges. On top of that, you have the Medicare budget cuts from the sequester and how that may impact operations, staff-cuts and implementation timeframes.
 
No matter how you approach large-scale implementation projects, building a strong foundation is key. Transitioning to an EHR, CAC or other new, complex technology requires the ability to analyze and understand a variety of documents and forms, clinical documentation quality and how they affect data integrity and patient record workflow.
 
Taking the time to prepare a best practice assessment of all key workflow elements, systems and processes can help your organization eliminate headaches before they happen and streamline the successful transition to an EHR/CAC.

Five Tips to Decrease DNFB

  1. Invest in periodic HIM operational/workflow assessments to pinpoint the source and cause of DNFB issues and identify possible solutions.
  2. Implement an ongoing tracking mechanism to monitor and report on DNFB improvements.
  3. Insure that PFS and HIM teams agree on how DNFB will be defined and measured and set realistic improvement targets and timelines.
  4. Enforce record completion policies. When possible, develop a plan to fill the gaps before regulatory changes, new technology installs or potential staffing shortages take their toll on productivity.
  5. Employ deficiency analysis prior to coding the chart (within the first 24 hours post discharge) to identify missing data earlier in the lifecycle of record processing to improve coder productivity and reduce the time spent searching for missing information.
