Fall is in the air, and a lot us are looking forward to the upcoming AHIMA Convention in sunny San Diego.  H.I.Mentors does not have a separate booth exhibit this year, but we still look forward to seeing many of you at the show.
I think we all are relieved to see the ICD-10-CM/PCS implementation date is back on track for October 1, 2015.  Many of you did not skip a beat in your transition plans and have kept the momentum going so as not to lose the hard work you have already put in getting your departments prepared.
It’s important that you use the extra time to take another look at budgets, staffing needs and IT and technology requirements.  Technology reviews, CDI assessments and training and I-9/1-10 coding audits can all help you set priorities and identify the gaps for making sure your organization is ready to roll with coding in 2015!  Quick Tips for Improving Documentation Quality:
5 Tips for conducting a forms inventory and management review as part of your ICD-10 transition program.

  1. If you haven’t already done so, put together a forms oversight committee and conduct regular meetings with all key department managers to build a detailed, all-inclusive forms inventory.
  1. Visit each department including ancillary facilities and conduct a physical forms inventory and map out forms workflow processes.
  1. Ensure that all forms are clearly categorized for easy identification, location and any changes required for Electronic Document Management readiness and to accommodate organizational programs or regulatory changes like ICD-10.
  1. Create a detailed forms master list of all forms by type and department. Look for duplications and eliminate forms where possible.
  1. Initiate tight “control” standards and written forms policies to help your organization manage and maintain forms in a best practice fashion and to implement and maintain a legal health record.

Solution of the Month: ICD-10: Keep the momentum and stay ahead of the game!

It is critical to use the extra time we have to redouble your efforts in preparing for ICD-10. If you haven’t done so already you need to take an inventory or how far along your organization is and where the major gaps are in the transition process. These might include accurate documentation, physician education, HIM education and training and IT systems or software readiness.
You will want to reset your preparation calendar to include the new dates for when coders are trained and ready. Establish milestones for dual coding in ICD-9 and ICD-10.  Conduct ICD-9/ICD-10 coding and clinical documentation audits. Begin or fine-tune physician education and documentation requirements. Plus, map out IT milestones and implementation schedules for new technology like code translation tools or computer-assisted coding software.
