Hospital Spotlight: Palmer Lutheran Health Center

Community collaboration at Palmer Lutheran has been instrumental in improving the readmission rates of patients.  By utilizing data to identify congested heart failure (CHF) patients as the most frequent readmits, the hospital constructed a program to drive improvements in the population. The local home health agency and Gunderson-owned clinic joined the hospital’s initial efforts of the program. The first order of business was to see what they were using for education.  After they became aware that each facility used different materials, standardization became the first necessary step for working collaboratively. This year's Patient Safety Conference will feature a presentation by Allison Massari. Our keynote speaker, Allison Massari knows that you have the power to transform lives. She has been a medical patient and educator, and understands health care intimately. As a burn survivor she draws on her personal experiences with empathic devoted healthcare workers. It's a passionate story of finding the internal strength and commitment for hope to flourish - and to inspire the collaboration of teams of medical personnel to profoundly demonstrate the power of authentic patient-provider relationships

IHC HEN Resources & Updates

  • This month, IHC held the HEN What's Up Webinar for hospitals that were able to attend. This webinar covered the enhanced metrics and reporting database changes to 2014. Listed below is the recording of the webinar and supporting documents:

  • IHC sent out the 2014 HEN Charters earlier this month to hospital CEOs. We encourage you to reach out to your hospital leadership to see completion of the charters. If you have any questions regarding this process, please contact meyerr@ihconline.org
  • The Iowa Department of Public Health and IHC will be hosting two regional Coverdell Stroke Action Team Learning Communities, featuring speakers from a local Stroke Action Team with expertise in stroke syndromes and the Miami Emergency Neurologic Deficit (MEND) Exam and a hands-on orientation to quality improvement tools that can be applied across the stroke continuum of care. The learning communities will allow personnel from across the care delivery system to work together to develop skills and relationships to improve the stroke care delivery and outcomes for patients. The learning communities will be held:
    • February 18, 2014, Iowa Hospital Association Education Center, 100 E. Grand Avenue, Des Moines, IA
    • February 20, 2014, Coralville Public Library, Coralville, IA
