The Roadmap for PFE

For this month’s spotlight we celebrate the great work that all of the HEN hospitals have done in working toward Patient/Person and Family Engagement (PFE) throughout Iowa facilities and communities. As we continue to strive together to expand statewide PFE efforts, we want to highlight a great resource that is available: The Partnership for Patients (PfP) Strategic Vision Roadmap for Person and Family Engagement (PFE).

The PfP Strategic Vision Roadmap for PFE was created by the national PfP PFE content developer, American Institutes of Research (AIR). The “Roadmap” is an excellent resource that outlines PFE as a critical component of hospital safety and quality, while providing meaningful breakdowns of what PFE is, how it relates to other key concepts (such as health equity!), and actionable steps we can undertake to support PFE at all levels of our work.

The Roadmap includes "preparation strategies" aimed at equipping patients, families, clinicians, healthcare staff, and leadership to be PFE champions. In addition, the roadmap provides a great framework to help guide hospitals to collect HEN PFE metrics and use data to develop hospital-specific
PFE strategies.

IHC Care Coordination Conference - Registration Open!

Join us as healthcare professionals from across the care continuum come together to drive partnerships and collaboration and learn about current transitions of care topics prevalent in today's healthcare environment.

Keynote speaker Amy Boutwell, President of Collaborative Healthcare Strategies, will give a keynote presentation on “Building Successful Cross-Continuum Collaboration.” Working at the intersection of all best practices and approaches to improve care transitions, without exclusive adherence to one particular model, Dr. Boutwell takes the best from what is known to be effective, practical and efficient in improving care transitions.

In a plenary session Mary DeVany will give a presentation on “The State of Telehealth in our Region.” Supporting organizations in their development and implementation of tele-health programs and services, she helps people develop an understanding of what they really need and figure out if and how telehealth can help meet that need.

The morning sessions will be followed by afternoon breakout tracks covering Team-Based Care, Data in Action and Social Determinants of Health.

HEN-HUB: Worker Safety
Interested in learning more about worker safety? Please join in on the conversation during our Worker Safety topic cycle on
the HEN-HUB!

Worker safety can include a variety of topics, including back safety, safe patient handling, and needlestick prevention. While not everyone may be following worker safety as a HEN focus area, please feel free to join in on the conversation as we can always gain great insight from our peers!

Our next two-week topic cycle will begin May 2, 2016. This cycle will be lead by Kirk Phillips, our Senior Health Informaticist, and he will discuss how to use data to monitor change! You don't want to miss this exciting topic cycle!
