SIM Spotlight

SIM-NQF Clinic Reporting Project
The IA SIM program goals have metrics and measures including four National Quality Forum (NQF) clinical measures. The NQF measures are collected in primary care clinics. The measures are rolled up with no patient or clinic identifiers, and submitted to the SIM Portal. These measures are related to population health efforts in the SIM Community Care Coalitions (C3). The clinic data along and other relevant data will result in analytics and reports for the C3s and clinicians to optimize transition of care and community health. The SIM and the C3s applaud the commitment and partnership efforts by the primary care clinics reporting NQF measures.

Association of State and Territorial Health Officials (ASTHO) Publishes Telehealth Resources Guide

The ASTHO Telehealth Resource Guide summarizes resources available to search for innovative ways to increase access to care for vulnerable populations and improve population health. In addition to describing key issues and resources, the guide highlights telehealth programs in seven states to demonstrate the variety and range of ways that states are coordinating and leveraging telehealth. This guide was developed by ASTHO with support from HRSA.

This has been made available online by Rural Health Value. This document catalogs and summarizes rural-relevant, value-based programs recently implemented by the Centers for Medicare & Medicaid Services (CMS), including the Center for Medicare & Medicaid Innovation (CMMI). Its purpose is to help rural leaders and communities identify HHS value-based programs appropriate for rural participation.

One of the core challenges in patient safety is learning about our mistakes and addressing them unblinkingly. Doing so requires that we are aware when we err, and sometimes this is not easy to do. For example, while surgical errors are usually obvious, the vast majority—probably more than 95%—of medication errors are unknown.
