Hospital Spotlight: Grundy County Memorial Hospital

Grundy County Memorial Hospital in Grundy Center, Iowa has implemented best practice guidelines to address VTE prophylaxis with every patient.  They have worked to create a new screening process to assess VTE risk.  They have also incorporated the screening tool into their Electronic Health Record (EHR). Beth Johanns, RN, BSN, CIC, and Clinical Outcomes Coordinator at Grundy County Memorial Hospital, has shared Grundy County’s story of implementation. 
“When the Partnership for Patients initiative began three years ago, VTE prophylaxis was not on the radar at Grundy County Memorial Hospital. Although we almost always met the SCIP measure for our total joint patients, we were not addressing it with every patient that stayed in our hospital.”

American College of Physicians 
This is a Choosing Wisely® list from the American College of Physicians that suggests providers perform a D-dimer measurement as the initial assessment for VTE, before imaging studies.

American Society of Hematology
This Choosing Wisely list, from the American Society of Hematology, has two recommendations in relation to testing for Thrombophilia and use of inferior vena cava filters in patients with VTE.

November Learning Community

The IHC Hospital Engagement Network (HEN) Learning Community will be held on November 18, 2014 at the Meadows Events & Conference Center in Altoona, Iowa.

This Learning Community will feature a presentation from Ellen McDermott, JD, from Outcome Engenuity in Plano, Texas on “A Just Culture: Building a Culture of Learning”.  Additional presentations will review topics related to physician engagement, patient and family engagement, and the Partnership for Patients (PfP) Hospital Engagement Network (HEN).  This Learning Community will also feature a patient and family engagement panel and an interactive "Ring of Knowledge" activity.
