Davis County Hospital

This month, Davis County Hospital in Bloomfield, Iowa, is featured for their work in implementing a Fall Reduction program, "More Balls, Less Falls," to improve patient safety by reducing falls within the hospital.
More balls, less falls; Davis county hospitals journey to fall reduction
The "More Balls, Less Falls" campaign began in September 2014 with a slow pitch softball tournament, including hospital staff and community members, to begin raising fall awareness. After the launch, a series of interventions were implemented throughout the hospital. The interventions were guided by a fall committee with representatives from each department.

Hospital-wide interventions included:
  • Gait belt distribution to easily accessible areas
  • Visual display showing progress in reducing falls daily (pictured above)
  • Post Fall Huddle Forms
  • White board visual aid for fall reduction techniques
  • Red lap blankets and non-skid socks for high risk patients
  • "Call, Don't Fall" painted ceiling tiles
  • "Falling Star" magnets for high risk falls
  • Fall risk assessments

Falls Community of Practice

IHC is offering the opportunity to participate as part of a Falls Prevention Community of Practice (CoP). As part of the CoP, participants will have access to ongoing education events, including online web series and in-person skill-building sessions.

Beyond available education, the CoP will serve as an interactive platform to connect participants across the state to drive collaboration and shared learning.

Please encourage all hospital employees to participate in this CoP to promote our PfP Campaign Goal of reducing preventable inpatient harm by 40%!

MBQIP - Reporting Update

Required MBQIP Quality Domain measures are reportable 4Q 2015 - 3Q 2016.  Due to CAHs not reporting some of these measures currently, HRSA has made this first year a planning and implementing year.  This means that hospitals must report at least one quarter by 3Q 2016 to be in compliance.  It is recommended that hospitals not wait until the last quarter to report.
EDTC measure data is reported to the IHC HEN Database for IA HEN CAHs.  There is a deadline for submission of each month. We strongly recommend hospitals follow the monthly reporting deadline schedule which is referenced in the IHC Database Metric and Measurement Toolkit on page 14.

Please note that because of the very different deadlines for MBQIP and HEN data, hospitals will likely enter MBQIP data prior to having all HEN data entered and completed. Due to this difference in data entry timelines and availability of all data, hospitals will not have the ability to “complete the month” with only MBQIP data entered. Months can only be “completed” when ALL MBQIP and HEN data has been entered. IHC will still have access to the entered MBQIP data even if the month has not yet been “completed”.

4Q 2015 EDTC data was due January 15th.  It was indicated to some CAHs the deadline was January 22nd. If you are unable to enter into the IHC HEN Database due post deadline (no later than January 22nd), you can send the data to Mary Beth Sorensen and she will see that is entered into the report prior to being submitted to HRSA. Please have this data to Mary Beth no later than COB January 27th. For all other months going forward please be sure to submit to the IHC HEN Database within the previously mentioned timeline.

Iowa Mission: Lifeline EMS 12-Lead Equipment Grant

Mission: Lifeline is an American Heart Association initiative to develop and enhance systems of care that improve patient care and ultimately reduce death and disability due to acute myocardial infarction (AMI). The first subset of AMI, that has the most evidenced based need for time critical systems of care implementation, is ST- elevation myocardial infarction (STEMI). To that end, Mission: Lifeline Iowa’s intent is to develop, support and enhance STEMI heart attack systems across the state.
The Iowa Mission: Lifeline EMS Equipment Grant seeks to enhance emergency medical response to STEMI heart attack patients in rural Iowa by funding requests for 12-lead equipment that will increase the ability to identify STEMI patients and relay critical medical information from the field to hospital providers.
