Registration Now Open for Iowa SIM Learning Community
You’ve saved the date, now it is time to save your seat at the State Innovation Models (SIM) Learning Community!  The SIM Learning Community will be hosted on November 9th at the Meadows Events & Conference Center in Altoona, IA. This event will be a great opportunity to hear from healthcare leaders and stakeholders from across the state, as well get an update on the national aims of SIM and hear about the next steps envisioned for Iowa.  In addition to learning sessions, attendees will also participate in active roundtable discussions.  These discussions will help solidify the information shared by speakers and provide participants the chance to share information back, looking at local practices, resources, and needed connections. Health IT–Enabled Care Coordination: A National Survey of Patient-Centered Medical Home Clinicians

Coordinating patient care is important for several reasons including the delivery of consistent guidance and recommendations, avoidance of unnecessary and/or duplicative testing, and ensuring timely access to services. A recent national survey showed that one-third of patients reported experiencing a gap in exchange of information between health care professionals involved in their care or between themselves and their health care professionals.

Model Programs & Promising Practices
Linking Medicaid Payment to Quality at the Nation's Community Health Centers  
The Commonwealth Fund
Along with other health care payers, state Medicaid programs are trying to develop new approaches to provider payment that promote better care at lower cost. In a new To the Point post, George Washington University’s Sara Rosenbaum and colleagues discuss how Medicaid programs are changing the way they pay community health centers, which cared for about one of five beneficiaries in 2015.

Through a Round Two State​ Innovation Model (SIM) grant from the Center for Medicare and Medicaid Innovation (CMMI), Maryland is developing a strategy to integrate care delivery for individuals who are dually-eligible for both the Medicaid and Medicare health care programs (dual eligibles).
