Over 400 in attendance at SIM Conference

The Iowa Healthcare Collaborative sponsored the Iowa State Innovation Model (SIM) conference on August 18.  The SIM initiative is a federally sponsored program through the Centers for Medicare and Medicaid Innovation (CMMI).  This four year program works to advance population-based healthcare transformation through alignment of provider, public health, and payer strategies focused on the Iowa Medicaid population.  A broad, diverse group with over 400 attendees representing the provider, public health and payer communities attended this meeting.

TeamSTEPPS Kick-Off

Thank you to those of you who were able to participate in our recent TeamSTEPPS Community of Practice  Kick-Off webinar!  For those of you who were unable to join us, reach out for a recording of this webinar.

HEN 2.0 Draft Measures

The purpose of the HEN is to achieve voluntary hospital participation and is an entity endorsed by the Centers for Medicare & Medicaid Services (CMS) as part of its Partnership for Patients (PfP) initiative. The Iowa Healthcare Collaborative was awarded the sole Iowa-based HEN contract to serve hospitals to advance the initial PfP campaign.

We provide training and technical assistance to facilitate hospital adoption of evidence-based clinical practices that improve safety and care coordination in 10 target areas.
