Physicians and Health Care Providers Continue to Improve Quality of Care, Lower Costs

The Centers for Medicare & Medicaid Services (CMS) today announced the 2015 performance year results for the Medicare Shared Savings Program and the Pioneer Accountable Care Organization Model that show physicians, hospitals, and health care providers participating in Accountable Care Organizations continue to make significant improvements in the quality of care for Medicare beneficiaries, while achieving cost savings. Collectively, Medicare Accountable Care Organizations have generated more than $1.29 billion in total Medicare savings since 2012.

Study Shows Patient Activation Improves Utilization Rates
The Commonwealth Fund
At baseline, poorer patient self-management—as identified by lower patient activation scores—was associated with a higher prevalence of chronic conditions, including depression and high blood pressure, as well as more ED visits and hospital admissions related to these chronic conditions.

Data for Change: How States Have Used APCDs to Drive Innovation 
NASHP
State-run all-payer claims databases (APCDs) are a critical public resource and serve a unique function in the current era of health care reform. APCDs, which are operating in 18 states, provide fair and equal access to independently validated data that can both support evidence-based policymaking as well as help patients navigate the health care system. These databases are also the only publicly accessible, independent resources available to analyze the billions of dollars spent on health care through private health insurance markets.

The National Quality Strategy (NQS) prioritizes community efforts and interventions to improve social, economic, and environmental factors that impact health, yet quality improvement and measurement have been overwhelmingly focused on the clinical care system. Evidence suggests that with a quarter of healthcare costs related to modifiable health risks, programs and policies that improve wellness and healthy behaviors could yield a net savings of $19 billion over 10 years.
