Join us June 3, 2015 as healthcare professionals from across the care continuum come together to drive partnerships and collaboration, learning about current transitions of care topics prevalent in today's healthcare environment.

Keynote speaker Jennifer Page, the inspirational and entertaining mother of Mini Darth Vader, will describe her family’s journey through the healthcare system. Her son, best known for his role as Darth Vader in the Volkswagen Super Bowl ad, was born with a congenital heart defect necessitating eight major surgeries before age eight. Her transformative healthcare journey has been heartfelt, eye opening and sometimes harrowing. Jennifer will share her many experiences to help participants better understand, identify and address many of the major challenges facing healthcare consumers today.
 
Clement (“Clem”) Bezold, PhD, Chairman and Senior Futurist, Institute for Alternative Futures (IAF) will present a keynote presentation on “The Social Determinants of Health in the Futures of Primary Care and Public Health”.
Trained as a political scientist, Clem Bezold has been a major developer of foresight techniques, including IAF’s unique “aspirational futures” approach, which uses scenarios to help clients not only to better understand what might happen.

The morning sessions will be followed by breakout tracks covering Community Team-Based Care, Data in Action and Patient and Family Engagement.

CUSP 4 MVP-VAP

We are reaching out to Iowa hospitals in regards to the CUSP 4 MVP-VAP project that is being led by the Johns Hopkins Armstrong Institute. The Iowa Healthcare Collaborative is the coordinating entity for the State of Iowa and we are currently recruiting for Cohort 3 which begins in July 2015. The Comprehensive Unit-Based Safety Program for Mechanically Ventilated Patients (CUSP 4MVP) rolled out their first Cohort in February 2014. The project is a guided program to help reduce ventilator -associated events (VAEs) in your ICUs. It will also be supported by the leading subject matter experts in the field, including Dr. Sean Berenholtz (Johns Hopkins Armstrong Institute) and Dr. Michael Klompas (Harvard Pilgrim Health Care Institute).
 
Your organizations are being invited to join CUSP 4 MVP–VAP: a critical care initiative to improve care for mechanically ventilated patients. Funded by the Agency for Healthcare Research and Quality (AHRQ) and led by national experts, this project aims to reduce preventable healthcare-associated infections and complications. By getting patients breathing and moving as soon as safely possible, together we can eliminate these preventable harms while reducing length of stay and improving hospital efficiency. Based on your current practices and improvement goals, your hospital teams can choose to focus on either daily care processes or early mobility initiatives.
