Hospital Spotlight: Stewart Memorial Community Hospital

This week's hospital spotlight features Stewart Memorial Community Hospital and their implementation of a multidisciplinary team to streamline the patient care transition process. Cindy Carstens and Jennifer Snyder of Stewart Memorial Community Hospital and Sheila Gregan of the Iowa Healthcare Collaborative wrote a summary article describing the transitions process. 

The following is from the article "Stewart Memorial Community Hospital identified multiple gaps in communication as patients transitioned through the care continuum.  They discovered that patient information was over looked, not current, or not delivered to the key people who needed that information. In response, they formed a multidisciplinary team with representation from the hospitals and clinics with the goal of improving the care transition process."

New Choosing Wisely Website

The Choosing Wisely® Campaign is an initiative aimed to help physicians, patients, and other healthcare stakeholders to think and talk about overuse of healthcare services. Choosing Wisely promotes wise choices by clinicians to improve healthcare outcomes, promotes patient-centered care that avoids unnecessary and even harmful interventions, and reduces the rapidly-expanding costs of the healthcare system.

The new Choosing Wisely website was created in partnership between IHC and Consumer Reports. Patients and providers can use this new website to access up-to-date Iowa specific resources about key health topics and evidence based recommendations for Iowans.

Registration Now Open!

The IHC Hospital Engagement Network (HEN) Learning Community will be held on November 18, 2014 at the Meadows Events & Conference Center in Altoona, Iowa.

This Learning Community will feature a presentation from Ellen McDermott, JD, from Outcome Engenuity in Plano, Texas on “A Just Culture: Building a Culture of Learning”.  Additional presentations will review topics related to physician engagement, patient and family engagement, and the Partnership for Patients (PfP) Hospital Engagement Network (HEN).  This Learning Community will also feature a patient and family engagement panel and an interactive "Ring of Knowledge" activity.  To view the Learning Community agenda for a complete list of speakers and presentations and for registration information, click this link!
