Self-management support videos

As part of a self-management support contract with the Ministry of Health, Health Navigator has created two videos to help New Zealanders manage their health conditions and medications.

Take Charge resources
The Take Charge resources are designed to assist with care planning and encourage people to identify areas they can work on themselves to improve their health. The resources are available in multiple formats:

 Take Charge: My Toolkit

This booklet is ideal for anyone wanting to get back in the 'driver's seat' of their health and improve the areas they can influence. Nearly everyone can improve some aspects of their health and wellbeing with the right knowledge, support and skills which this booklet will help you learn. 

  • Can be worked through on your own and then with you healthcare team.
  • Ideal assessment and care planning tool for nurses, GPs, allied health staff and health coaches.
  • 40 pages.


 Take Charge: My Health Plan

  • This is a shorter version of the Take Charge Toolkit.
  • Ideal for those who want a short version focusing on the care plan sections.
  • Ideal assessment and care planning resource for use by health providers as above.
  • 9 pages.

App reviews

New app reviews are being added to the Health App library on a regular basis. Recent additions include:
  • My Asthma app – created by the Asthma and Respiratory Foundation NZ for people wanting to learn about asthma and create an asthma action plan.
  • mybc app  – created by Melon Health, in partnership with the New Zealand Breast Cancer Foundation, to help New Zealanders affected by breast cancer to connect with others, share experiences, ask questions and get support.
New app reviews are being added to the Health App library on a regular basis. If there are any categories that you would like added to the health app library or you would like to review a favourite health app for us email

Studies show learning is enhanced when patients are given quality written or online information to support their retention and understanding following a consultation or admission. The Health Navigator NZ website is designed to make it easy for your clinical teams to give patients and clients relevant NZ information and self-care resources. 

You can print out topic sheets, or jot down topic suggestions on our wallet cards for patients to look up online at home. We also have posters and recently updated DLE rack cards that are ideal for outpatient waiting room
